form with a check made out to "Quilt Tours of Las
Vegas" at: P.O. Box 50225, Henderson, NV 89016. A deposit
of $25 with this form will hold a seat for you.

Registration
and Cancellation Information
- Deposit of $25 with
registration. (Full refund of deposit if written
cancellation received on or before 12/1/08)
- Balance due on December 1,
2008.
- After December 1 until January
7, 2009, you may cancel with a 50% cancellation fee. The
cancellation must be made in writing and received by us on
or before January 7, 2009.
- No refunds after January 7,
but you may transfer your reservation to a friend. Any
financial arrangements must be made between you and your
friend. Please have them register and indicate in the
comments area that they are taking over your seat on the
tour.
- Quilt Tours of Las Vegas
reserves the privilege of canceling the tour if an
insufficient number of registrants are received. Should
Quilt Tours of Las Vegas need to cancel the tour; a full
refund will be made within 5 days of notification.
